A form is provided by the life insurance carrier to obtain an individual's declaration of personal, occupation, health, financial, and other information. The information provided by the insured (and typically completed by the life insurance agent) forms the basis on which a life insurance company will make an offer to provide coverage. The application becomes a part of the legal contract of insurance, and the insurer is generally allowed to challenge misstatements if death occurs within 2 years of the policy issue.
